Parents! Below you will find a medical release form as well as a photo consent form.
The medical release is extremely important, and will be used to notify you in case of an emergency, and for basic medical care and/or treatment. Please fill it out completely. You can have your student return it during youth group, mail it (975 Rolling Acres Road Lady Lake 32159), fax it (352-753-3982), or drop it off at the church.
The photo consent form is used for permission to publish pictures of your child on our website, used for print or on our facebook page. We will be doing lots of great things over the year and we like to show our congregation what we are up to throughout the year. If you consent, please return this form with the medical release.
Both forms will be good for the 2011 calendar year.
If you have any questions regarding any youth event please contact Holly Reimer at hollyr@northlakepc.org or at the church office 352-753-8484.
